I want to apply for registration – what do I do?

Once you’ve completed the Family Doctor Services Registration Form (GMS1W) and the New Patient Questionnaire, return them both to the practice and your application will be considered by the practice partners. Please note that your application cannot be considered until we’ve received both forms along with proof of your address.

All information you supply will be handled and stored in line with current professional guidance and relevant data protection / GDPR guidance.


What happens next?

Once you’ve completed and returned all the paperwork, we’ll double check that we have everything required to provide safe treatment. If we have any queries, we’ll get in touch and once complete we’ll get you registered and will take forward your ongoing care and prescriptions as appropriate.

When booking an appointment, you will be asked the reason for ALL appointment requests and will be actively signposted / redirected to other services e.g. pharmacy, optician, dentist etc where they are more appropriate.

  • We actively support the Choose Well (https://www.choosewellwales.org.uk), Choose Pharmacy, Minor Illness and Minor Injury schemes running in the area and we will encourage you to use them where they are a more suitable choice.
  • Your appointment will be with the most appropriate member of our healthcare team for your condition and is more likely to be with one of our nursing colleagues than with a GP.
  • Sleeping tablets and similar medicines – we follow national patient safety guidance on these and do not support their long term use. As practice policy, we will discontinue or actively reduce them where clinically appropriate.
  • We support Health Board prescribing guidelines on not prescribing products which are readily available over the counter from pharmacies and supermarkets e.g. cough syrup and simple painkillers such as paracetamol and ibuprofen.
  • We use pre-appointment information gathering measures (computer based) to ensure that our available appointments are used as effectively as possible.
