I want to apply to Register at the practice – what do I do?
Once you’ve completed the Family Doctor Services Registration Form (GMS1W) and the New Patient Questionnaire, return them both to the practice and your application will be considered by the practice partners. Please note that your application cannot be considered until we’ve received both forms along with proof of your address, which must be dated within the last 3 months.
All information you supply will be handled and stored in line with current professional guidance and relevant data protection / GDPR guidance.
Under 16’s
Children under the age of 16 whose parent/guardian is registered with the practice/registering at the same time will need to provide either:
- Original birth certificate or a certified copy
- Passport
If you are unable to provide any of the above documents, please speak to a member of the reception team who will be able to discuss alternative documents.
What Happens Next?
Once you’ve completed and returned all the paperwork, we’ll double check that we have everything required to provide safe treatment. If we have any queries, we’ll get in touch and once complete we’ll get you registered and will take forward your ongoing care and prescriptions as appropriate.
Temporary Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP Practice for 14 days. This only applies to immediate or ‘urgent’ care. After 14 days you will need to register as a temporary or permanent patient.
GMS3 Temporary Patients Registration Form
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with the practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as temporary patient although they do have an obligation to offer emergency treatment, as above. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
Contact Information & Consent
Our Practice uses a variety of methods to contact you, these include letter, email, text and telephone.
The systems that we use are secure and allow us to proactively contact patients regarding the following:
- Regarding test results.
- Reminders such as when your annual review is due.
- To request that you contact the surgery.
- To contact you if your appointment needs to be rescheduled.
- To update you regarding services that are available at the practice.
By providing us with your mobile number and email address, we automatically assume that you consent to us contacting you via text and email.
Should you wish to opt out of receiving texts or email at any time, please let our Reception Team know. We will only contact you if it is necessary.
**Please Note – If you move to a new address outside our Practice Catchment Area, you will need to register with another practice.
